I don’t know where do you see low voltage traces close to mains voltage. Trace from R8 to the opto is 2.64. How far do you think it should be to meet your standards? Conformal coating is telling you anything?

It’s a 750W pump. Since it is running on 220V, steady current would be ~3.5A, so inrush current could be as high as 20A, according to some sites I’ve just found. Doesn’t sound awesome.

A 25A relay seems perfect for a 22A load. If the relay is really built for 25A and not only marked as one. What use had a maximum rating when you could never really use it?

I came here to post those two points! The corollaries to 1) are that should something go wrong, you have a chance if there is someone else there who knows CPR. I like mandatory CPR training for lab workers because of this. AEDs are good too. Rules against playing with things that can shock you while working alone are also good, but much harder to follow–I’ve spent decades working alone in research labs. CPR fails way more often than not, but electrical shocks are an important exception to that rule. And you almost got the extra credit for correctly spelling arrhythmia. 
I would also use a solid state relay. And I would think about gluing the glass plate with high temp silicone to the PCB. This vastly improves thermal contact and so heat transfer which reduces peak temp and thermal lag in the control loop. Also it prevents exposed live wires even in case the glass breaks.
Yup, switched off but still plugged in, that way the case is still earthed. Assuming a sensible 3-pin plug, or whatever the hell they do in places with just 2.
